*Registration for Walk and/or Zoom is included in all donations and is $36 per household. The registration fee represents the value of the event and is not tax-deductible.
The fair market value of an individual ticket to the Walk and Celebration is $36. The fair market value of such tickets represents the portion of your gift that is not tax-deductible. Charitable deductions are allowed to the extent provided by law. Hadassah shall have full dominion, control and discretion over all gifts (and shall be under no legal obligation to transfer any portion of a gift to or for the use or benefit of any other entity or organization). All decisions regarding the transfer of funds to or for the benefit of any other entity or organization shall be subject to the approval of the Board or other governing body of Hadassah. Donors are encouraged to read Hadassah’s Charitable Solicitation Disclosure Statement.
*COVID WAIVER AND RELEASE: By registering for or attending this event/meeting, I certify that I will not attend the event/meeting if I have (i) temperature of 100.4˚F or higher, (ii) any symptoms of COVID-19, (iii) tested positive or been diagnosed with COVID-19 prior to the event/meeting without quarantining and/or isolating for the period of time then recommended by the CDC, or (iv) been exposed to anyone diagnosed with COVID-19 prior to the event/meeting without quarantining and/or isolating for the period of time then recommended by the CDC. I further hereby acknowledge and agree that I understand and will follow all guidelines applicable to this event/meeting.
I understand that COVID-19 has been declared a pandemic and that a communicable infectious disease such as COVID-19 is highly contagious. I also understand that my attending an event/meeting sponsored by Hadassah, The Women’s Zionist Organization of America, Inc. (“Hadassah”) or any Hadassah unit, or similar group, will result in my interacting with other people, including but not limited to, those attending the event/meeting, event and venue staff and other individuals at the location in which the event/meeting may take place. For this reason, there is no way to guarantee, despite the best efforts of the individuals holding the event/meeting, that such other people have received COVID-19 vaccinations and/or booster shots, do not have, or have not been exposed to, COVID-19. As such, I, for myself, my next of kin, family members, heirs, representatives, agents, successors and assigns, hereby release Hadassah, its chapters, regions, units, related entities and affiliates and their officers, directors, employees, volunteers, representatives, agents, successors and assigns and anyone claiming through them (collectively, “Hadassah Releasees”), in their individual and/or organizational capacities from any and all claims, damages, losses, liabilities, obligations, and causes of action of any nature and kind (including reasonable attorneys’ fees), known or unknown, which I have, ever had or may in the future have against Hadassah Releasees arising out of or relating to any exposure to or illness resulting from COVID-19.
This Waiver/Release shall be governed by and enforceable in accordance with and shall be subject to the laws of the State of New York without regard to its conflict or choice of law provisions. I hereby consent to the exclusive personal jurisdiction and venue of any state or federal court located within the City of New York and waive any objection to jurisdiction and venue of any action instituted hereunder and agree not to assert any defense based on lack of jurisdiction or venue.
